Open distal anastomosis technique in acute type A aortic dissection

چکیده
Background Acute ascending aortic dissections is the most common catastrophe of the aorta that requires emergency surgery. There is still a debate regarding the optimal surgical approaches for construction of the distal anastomosis in replacing the ascending aorta. The purpose of this study was to evaluate early clinical outcomes of two different surgical techniques: open distal anastomosis in hypothermic circulatory arrest (OHCA) compared to anastomosis with clamped aorta while continuing on extracorporeal circulation (CECC).

Open and closed distal anastomosis for acute type A aortic dissection repair: early and long-term outcomes from a contemporary series of 204 patients.
OBJECTIVES The current consensus favours an open distal anastomosis for aortic dissection repair. A small number of experiences have compared early and long-term outcomes between closed and open distal anastomosis in the setting of acute aortic dissection. METHODS We reviewed our experience in 204 patients who underwent repair of spontaneous acute type A aortic dissection between January 2000...
متن کاملTime and risk analysis for acute type A aortic dissection surgery performed by hypothermic circulatory arrest, cerebral perfusion, and open distal aortic anastomosis.
BACKGROUND Hypothermic total circulatory arrest, retrograde or antegrade cerebral perfusion, and open distal anastomosis are important stages of surgical management and cerebral protection for acute type A dissections. Among the factors that influence survival are the transfer time to hospital from the onset of symptoms, in-hospital transfer time to operation, organ malperfusion, preoperative r...
متن کاملAdventitial inversion with graft telescopic insertion for distal anastomosis in acute type a aortic dissection.
PURPOSE One current method of anastomosis in aortic dissection type A is the adventitial inversion technique. To improve hemostasis at the anastomotic site, we have developed a novel technique for distal anastomosis involving adventitial inversion employing graft telescopic insertion. METHODS The adventitia was inverted into the aortic lumen and the anastomosis with a Dacron tube-graft was ma...

OBJECTIVES We review cases of pseudoaneurysm formation of the graft anastomosis sites following repair of type A acute aortic dissection by our original leak-proof technique for dissected aortic wall reinforcement with xenopericardium and gelatin-resorcinol-formalin (GRF) glue. CASES A 47-year-old male presented inferior acute myocardial infarction with bradycardia and cardiogenic shock 34 mo...
متن کاملSuccessful Repair of Type a Aortic Dissection in a Term Pregnancy: A Case Report
Stanford type A acute aortic dissection (AAD) is a very rare complication, with potentially lethal consequences in pregnancy. In fact, pregnancy has been regularly associated with the possibility of aortic. dissection in almost half of young women. Herein, we present the case of a 38-year-old woman in her 37th week of pregnancy. The patient’s medical history was indicative of G4L2Ab1(4 gestaiti...
